A revolutionary lung transplant machine could save four in five donated lungs that currently go to waste in England and Wales, according to new draft guidance from the National Institute for Health and Care Excellence (Nice). The UK has one of the poorest lung transplantation rates globally, with only 23% of donated lungs being used. This machine, called the XVIVO perfusion system, warms organs to body temperature and pumps a nutrient-rich fluid through them while simulating breathing.
How the Lung Transplant Machine Works
The XVIVO system keeps donated lungs alive outside the body by mimicking natural conditions. It warms the organs to body temperature, pumps a fluid that replicates blood plasma, and ventilates them to imitate normal breathing. This allows transplant teams to monitor lung function, treat infections, drain excess fluid, and reinflate collapsed areas. According to Nice, this technology could rescue hundreds of organs that are currently discarded due to quality concerns or logistical issues.

Current Donor Lung Utilization Rates
| Region | Donated Lungs Used | Patients on Waiting List |
|---|---|---|
| England & Wales | 23% | 170 (including 6 children) |
| Ideal with XVIVO | Up to 80% | Significantly reduced |
The table above highlights the stark disparity. Currently, up to a fifth of adults awaiting an urgent lung transplant die within a year. Most adults wait a year, and some wait two years or more. The new lung transplant machine could cut these waiting times dramatically.
Key Benefits of the XVIVO Perfusion System
- Rescues 80% of currently unused donated lungs
- Allows real-time assessment of organ viability
- Reduces transplant waiting list mortality
- Can be used in paediatric cases for the first time
- Improves equity across transplant centres of all sizes
Clinical Impact and Cost Effectiveness
Draft Nice guidance recommends a four-year trial of the XVIVO system in the NHS to assess its effectiveness and value for money. The machine has already been trialled at three adult transplant centres, but until now Nice had not endorsed it for paediatric use or for all centres. Dr Anastasia Chalkidou, healthtech programme director at Nice, stated that every donated lung is a precious gift, and this technology ensures more can be used.
FAQ
What is the XVIVO perfusion system?
It is a machine that keeps donated lungs alive outside the body by warming them and pumping a fluid that mimics blood plasma while ventilating them. This allows doctors to assess and treat the lungs before transplant.
How many donated lungs could this machine save?
Nice estimates the machine could rescue four in five donated lungs that are currently unused, potentially unlocking hundreds of organs for transplantation each year.
Why are so few donated lungs used in the UK?
Donated lungs are often declined due to organ quality issues, but also because of staffing shortages and limited theatre capacity. The XVIVO system helps overcome quality concerns by keeping lungs viable longer and enabling better assessment.
The draft Nice guidance marks a significant step forward. If adopted, the lung transplant machine could transform transplant outcomes in England and Wales, giving hope to the 170 patients currently waiting. As the technology is evaluated over the next four years, it may set a new standard for organ preservation worldwide.
