The lung perfusion machine that keeps organs alive outside the body could dramatically increase NHS transplants, saving four in five donated lungs currently wasted. This innovative technology, recommended by Nice, offers new hope for hundreds of patients on the UK transplant waiting list.

Why the UK Needs More Lung Transplants

The UK has one of the poorest lung transplantation rates globally. Latest NHS Blood and Transplant figures reveal 170 patients, including six children, are waiting for new lungs. Most adults wait a year or more, and up to a fifth of urgent patients die within 12 months of joining the list.

Currently, only 23% of donated lungs are used. Organs are often declined due to quality concerns, staffing shortages, or theatre capacity issues. This waste is tragic when every lung could save a life.

How the XVIVO Perfusion System Works

The XVIVO machine warms donated lungs to body temperature and pumps a fluid mimicking blood plasma through them. It also ventilates the lungs to simulate normal breathing, allowing transplant teams to monitor function, treat infections, drain fluid, and reinflate collapsed areas.

This organ preservation technology extends lung viability for up to six hours, unlocking hundreds of organs that would otherwise be discarded. Draft guidance from the National Institute for Health and Care Excellence (Nice) recommends a four-year NHS trial to assess its effectiveness and cost-efficiency.

Benefits for Transplant Centres and Patients

Until now, the machine was only trialled at three adult centres. Nice's draft guidance aims to make it available across all transplant centres, regardless of size or location, and includes paediatric cases. Dr Anastasia Chalkidou, Nice's healthtech programme director, says the technology should ensure more donated lungs remain usable.

This innovation could reduce waiting times, lower mortality rates, and improve quality of life for patients with severe lung disease. It also optimises the use of precious donor organs, honouring the gift of donation.

Comparison: Current vs. XVIVO-Assisted Transplants

Metric Current Standard With XVIVO Machine
Lungs used from donations 23% Up to 80% (estimated)
Viability time outside body 4-6 hours Up to 6 hours with monitoring
Treatment of infections Limited Possible during perfusion
Access across centres Only 3 adult centres All centres, including paediatric

Key Takeaways

  • The XVIVO perfusion system could rescue 4 in 5 unused donated lungs.
  • It extends organ viability and allows real-time assessment and treatment.
  • Nice draft guidance supports a 4-year NHS rollout to all transplant centres.
  • This could significantly reduce UK lung transplant waiting lists and deaths.

FAQ

What is the XVIVO lung perfusion machine?

It's a device that keeps donated lungs alive outside the body by warming them, pumping a plasma-like fluid, and ventilating them. This allows transplant teams to assess, treat, and preserve lungs for up to six hours, increasing their usability for transplantation.

How many more lung transplants could this enable?

Nice estimates the machine could help rescue four in five donated lungs that are currently unused, potentially unlocking hundreds of additional organs for transplants each year in England and Wales.

Will the machine be used for children?

Yes, the draft Nice guidance includes approval for paediatric cases, ensuring that children waiting for lung transplants can also benefit from this technology across all NHS transplant centres.

The NHS lung transplant innovation represents a major step forward in organ preservation. With wider adoption, it promises to save lives, reduce suffering, and make the most of every generous donation. If you or a loved one are affected by lung disease, discuss this emerging option with your transplant specialist.
