A New York City med spa owner has admitted to causing a botulism outbreak by administering bootleg Botox, highlighting the dangerous consequences of illegal cosmetic procedures. Klara Shuster, also known as Klara Chrzuszcz, pleaded guilty in federal court to conspiracy to commit smuggling and other charges after she illegally imported mislabeled Botox from Asia and injected it into clients without medical supervision. One client developed botulism, a potentially fatal condition.

The Botulism Outbreak and Legal Consequences

Shuster, an esthetician, ordered a drug containing botulism toxin over the internet from Korea and dispensed it to clients in Manhattan between February 2020 and October 2025. During her August 3 plea hearing, she admitted, "I imported mislabeled Botox illegally to this country... And then I dispensed it to clients mislabeled, and I knew it was against the law." She also acknowledged doing so "without the prescription and without any medical supervision."

US District Court Judge John Koeltl questioned whether she "dispensed a prescription drug containing botulinum toxin that you asserted was a particular drug, but was not that genuine drug." The client who received the substance was hospitalized for botulism, which attacks the body's nerves and can be fatal. Shuster's involvement in the smuggling plot spanned over five years, according to the US attorney's office for the Southern District of New York.

Risks of Bootleg Botox and Illegal Med Spas

Botulism is a rare but serious illness caused by a toxin that attacks the nervous system, leading to difficulty breathing, muscle paralysis, and even death. When Botox is administered by unlicensed individuals or sourced from unregulated suppliers, the risk of contamination, incorrect dosing, and severe adverse reactions skyrockets. Legitimate Botox is a prescription drug that must be administered under medical supervision.

Illegal med spas often lure clients with lower prices, but the savings come at a dangerous cost. Counterfeit products may contain harmful substances or incorrect concentrations of botulinum toxin. Additionally, without proper medical oversight, complications like botulism can go unrecognized and untreated.

How to Protect Yourself: Legit vs. Bootleg Botox

Before undergoing any cosmetic injectable treatment, verify the provider's credentials and the product's authenticity. Here's a quick comparison:

Factor Legitimate Botox Bootleg Botox
Source FDA-approved, licensed distributor Unregulated, often imported illegally
Prescription Required Yes No
Medical Supervision Administered by licensed professional Often none
Safety Record Extensive testing, low risk of serious complications High risk of infection, botulism, and other severe reactions
Cost Higher, reflects quality and safety Lower, but dangerous

Key Takeaways for Consumers

  • Always verify credentials: Ensure the injector is a licensed medical professional (doctor, nurse, or physician assistant) trained in cosmetic procedures.
  • Ask about the product: Legitimate providers will show you the vial and allow you to verify the brand and lot number.
  • Beware of too-good-to-be-true prices: Deep discounts may indicate counterfeit or illegally imported products.
  • Report suspicious providers: If you suspect a med spa is operating illegally, contact your state medical board or the FDA.

FAQ

What is botulism and how is it linked to Botox?

Botulism is a rare but serious illness caused by a toxin that attacks the body's nerves. Botox is a purified form of botulinum toxin type A, which is safe when administered in tiny, controlled doses by a licensed professional. However, if the toxin is contaminated, incorrectly dosed, or injected without medical supervision, it can cause botulism.

How can I tell if a med spa is legitimate?

Legitimate med spas employ licensed medical professionals, use FDA-approved products from authorized distributors, and require a prescription for injectables. They will also be transparent about the products used and allow you to verify their credentials. If a provider avoids questions or offers unusually low prices, be cautious.

What should I do if I experience symptoms of botulism after a cosmetic procedure?

Seek immediate medical attention if you experience difficulty swallowing or breathing, blurred vision, drooping eyelids, slurred speech, or muscle weakness after receiving Botox or other injectables. These are signs of botulism and require urgent care. Also report the provider to your state health department or medical board.
