Sydney emergency departments are under unprecedented strain this winter, with health officials urging the public to avoid the ER unless life might be in danger. A record day for winter presentations has prompted NSW Health to launch a permanent command centre to manage peak demand and redirect patients to appropriate care.

Record Winter Day for Sydney Emergency Departments

On Monday, just under 10,000 people presented to emergency departments in Sydney, the biggest ever recorded winter day in history. This was around 1,000 more than the average, according to NSW Health Minister Ryan Park. The surge has placed immense pressure on hospital staff, particularly in west and south-west Sydney.

Ambulance callouts also hit 4,100, far exceeding predictions. The state government has now established a permanent command centre to coordinate responses across 17 local health districts and NSW Ambulance, aiming to redirect patients away from overcrowded EDs.

Why Are Emergency Departments So Busy?

Winter typically brings a rise in respiratory illnesses, including influenza, Covid-19, and RSV. The latest NSW Health surveillance report shows influenza and RSV at moderate levels, with Covid-19 low but present. ED presentations for influenza are around 400 per week, up from recent weeks but below the peak of 1,000 in July 2025.

When to Avoid the Emergency Department

Health Minister Ryan Park emphasized: “There is no need to go to an emergency department unless it is an emergency.” Instead, residents are urged to use Healthdirect, a free phone service connecting you to registered nurses for advice and referrals, or visit urgent care services that operate from 8am to 8pm.

Alternatives to the ER

  • Healthdirect – Call 1800 022 222 for 24/7 nurse advice.
  • Urgent care clinics – For non-life-threatening issues like minor infections or sprains.
  • Your GP – Book an appointment for ongoing conditions or prescriptions.
  • Pharmacist – Get advice on over-the-counter remedies for cold and flu symptoms.
