Australia is facing another devastating domestic violence crisis after two women and two girls were allegedly murdered in four days. The deaths of Lavanya Chappa, Jana Armstrong, Layla Jeffery, 13, and a 17-year-old Yolngu girl have reignited urgent calls for action from experts and advocates nationwide.
Why the Crisis Continues
Despite repeated tragedies, experts say the problem is not a lack of knowledge but a failure to implement reforms. Katherine Berney, a policy expert on gender-based violence, notes, “We already have more than 1,000 recommendations. The knowledge gap isn’t there. There’s an implementation gap.”

This pattern of outrage followed by inaction has persisted for years. In 2024, researchers reviewed recommendations from Queensland and New South Wales death reviews and found that only 16% of recommendations had been properly enacted over more than a decade.

| Metric | Data |
|---|---|
| Women killed in four days (latest crisis) | 2 women, 2 girls |
| Recommendations from death reviews implemented | Only 16% |
| Years since national blueprint called “time for action” | 17 years |
| Number of recommendations available | Over 1,000 |
The Cycle of Outrage and Fatigue
Experts describe a frustrating cycle where tragedies spark public outrage, governments respond with performative measures, and lasting reforms are delayed. Dr. Emma Buxton-Namisnyk, a co-author of the study, questions whether government responses are “performative,” aimed at managing public concern rather than effecting real change.
Most Australian states have death review systems, but their recommendations often go unimplemented. This systemic failure leaves women and children vulnerable.
What Needs to Change
- Full implementation of existing recommendations from death reviews
- Increased funding for prevention programs and support services
- Accountability measures for government agencies
- Community education to break the cycle of violence
- Stronger legal protections for victims
