Stroke risk factors for Black African and Caribbean populations are rising faster than for white people, according to a new study from King’s College London. The research, published from the South London Stroke Register, tracked over 8,500 adults for 30 years and reveals alarming trends in hypertension and diabetes prevalence among ethnic minority groups.

How Stroke Risk Factors Differ by Ethnicity

The study found that diabetes was about twice as common in Black Caribbean and Black African stroke patients compared with white participants. High blood pressure was 29% more prevalent in Black Caribbean individuals and 47% more prevalent in Black African individuals. These stroke risk factors are not only more common but also increasing more rapidly.

Rising Diabetes Rates Over Three Decades

Diabetes rates climbed for all ethnic groups between 1995 and 2024, but the increase was steepest among Black African participants. The prevalence rose from 21% in the first decade to 41% in the last decade — a near doubling. For Black Caribbean participants, the rate increased from 25% to 34%, while white participants saw a smaller rise from 14% to 22%.

Younger Strokes and Missed Warning Signs

Black people were also twice as likely to experience a stroke without a prior diagnosis of any risk factor — 12% compared with 6% for white individuals. This suggests that stroke prevention programs may be failing to catch at-risk individuals early enough. Dr. Eva Emmett, lead author, noted that NHS health checks begin at age 40, but this may be “too late for higher-risk groups who often experience stroke at a younger age.”

What This Means for Prevention

The findings call for targeted, earlier screening and lifestyle interventions. High blood pressure and diabetes are modifiable risk factors, but they must be detected and managed before a stroke occurs. Community-based education and culturally tailored health campaigns could help close the gap.
