Weight-Loss Drug Costs: A Regressive Tax on Being Thin

The high cost of weight-loss medication is a regressive tax on being thin, as the outlay outweighs any savings on food costs for all but the highest earners. According to a new analysis by consultancy Baringa, only individuals with discretionary income of nearly £100,000 per year actually save money after covering the £1,200 annual cost of GLP-1 medication. This financial burden is reshaping the conversation around obesity treatment and health inequality.

Why GLP-1 Drugs Are a Regressive Tax on Being Thin

Paddy Winters, a partner at Baringa, explains that “GLP-1s risk becoming a driver of inequality.” The term “regressive tax” refers to a levy that takes a larger percentage from lower-wage earners’ income compared with wealthier individuals. In this case, the fixed cost of the medication hits lower earners disproportionately harder, while the benefits—reduced grocery bills—only materialize for the affluent.

The Financial Breakdown

Baringa’s analysis reveals that a GLP-1 user with £39,000 left after tax and essentials will spend £481 less per year on food. However, it is not until someone has £97,500 left to spend after tax and other outgoings that their grocery spend is reduced by the full £1,200 annual cost of the drug. For everyone else, the medication cost exceeds any food savings.

Discretionary Income (After Tax & Essentials) Annual Food Savings Net Cost of GLP-1
£39,000 £481 £719
£60,000 £800 £400
£97,500 £1,200 £0
£120,000 £1,500 -£300 (savings)

Additionally, Winters notes that users are spending more on vitamin supplements and personal care products like hair care to manage side effects. “In effect, GLP-1s become a regressive tax on being thin,” he said.

The Cycle of Start, Stop, Regain

The financial strain is already causing some users to cancel their prescriptions after a few months. “But the weight comes back once you stop,” Winters warns. This creates a pattern of start, stop, regain, and return, which may drive people to take on debt to fund the cycle. This is particularly concerning given that 14.3 million adults in England live with obesity, according to the King’s Fund.

Health Inequalities Worsen

The King’s Fund recently reported that the government “needs to ensure that the people who benefit from [weight-loss medication] most are not priced out of accessing them, and that we don’t end up with wider health inequalities.” Without intervention, these drugs could exacerbate existing disparities, making it harder for lower-income individuals to manage obesity effectively.

Key Takeaways

  • GLP-1 weight-loss drugs cost around £1,200 per year, which only high earners can offset with food savings.
  • Lower-income users face a net financial loss, making the drug a regressive tax on being thin.
  • Side effects lead to additional spending on supplements and personal care, increasing the burden.
  • Many users stop medication due to cost, leading to weight regain and a cycle of yo-yo dieting.
  • Policy changes are needed to prevent widening health inequalities.

FAQ

What is a GLP-1 drug?

GLP-1 receptor agonists are medications used to treat type 2 diabetes and obesity by regulating appetite and blood sugar. Examples include semaglutide and liraglutide.

Why are GLP-1 drugs considered a regressive tax?

Because the fixed annual cost of £1,200 takes a larger percentage of income from lower earners, and the food savings only offset this cost for those with high discretionary income, similar to a regressive tax.

How much can users save on food bills?

A user with £39,000 disposable income saves about £481 annually on food, but only those with £97,500 or more see savings equal to or greater than the drug cost.
