Description of measurements
We had purchased some 12' long 2x4 vehicles since the space we are working with is 13' long. Then we placed them and played with the space between them until we found something that looked balanced. We set up on 24"x24" blocks with 6" spacing in the center and between rows. This left approximately 12" on each side.
Finished projects:

Procedure
Step 1: Wooden molds

We cut several 24" long pieces, as well as several 3" pieces to act as spacers. We cut them to 3" because each board is 1 1/2" wide.
We placed our 12' board down and used the nail gun to attach the 24" boards together. The boards in the photo above that are parallel to the doorway are spacers. We use the same spacers all the time to ensure uniformity.
Step 2: Dig the surface

Once that was completed, it was time to dig more. We wanted the spacers to be embedded in the ground and were working with a natural slope. I won't go into too much detail because it's very specific to our space we're working with, but basically we needed to bury one side of the form.
Step 3: Fill with gravel

We picked up a few bags of gravel and poured about 1" of it into the bottom of each square. We mixed the concrete in our wheelbarrow according to the instructions on the bag
Step 4: Add concrete

We use a piece of wood on both sides of the form and use a back and forth sawing motion to get everything level.
we checked it with a rubber float to smooth things out. Finally, we used an edger to make clean edges on each square.
Step 5: cover with plastic

We covered everything with a plastic sheet and left it overnight. The next day was the removal of the form. We started by cutting the wood between each square with the reciprocating saw so we could remove the form square by square. They were difficult to remove and we ended up using a pry bar to lift the form while it bounced up and down on the concrete.
Step 6: Final finishing
It looked pretty good, but we still had to deal with the rough, ugly pavers we had made before we got the hang of things. We decided to pick up a bag of Portland cement and apply athin layer. This is different from concrete mix as it does not have sand or gravel mix.
